Membership Cancellation Form

By submitting the following membership cancellation request, you agree to cancel your membership immediately at the end of your current billing cycle and stop all ongoing charges moving forward.

Cancelling your membership will stop your access from receiving any more membership benefits after the end of your monthly billing cycle. Your current membership credit will remain unaffected, and you will be able to keep any remaining membership benefits (such as your free microneedling treatment) if you cancelled past your original yearly obligation (including any renewals).

You understand and agree that your original membership contract required an original minimum obligation of a 1 year monthly billing cycle from the day you had started your membership. If you cancel before this one year original contract, you agree to be charged for any original membership benefits you may have received, as if you were not a member at the time you received these benefits. This will be calculated by Glow Aesthetic Center staff and confirmed with you by a written email prior to proceeding on your cancellation.

If you cancel past your one year original membership, you agree to rescind any additional benefits you may have received after your 1 year renewal (like any complimentary microneedling treatments) or that you pay for the original pricing of the additional membership benefits (your complimentary microneedling) if the benefit was already used prior to the end of your 1 year renewal. This would not include any discounts you may have received due to already filling your original year obligation.
